This session will covers the description of the basic causes of incidents in the workplace, reporting these incidents, preparation of an investigation kit, Identify the members of an investigation team, Follow a systematic investigation process, Identify the root cause of an incident, Make recommendations to prevent recurrence of an incident.

Staff member work is required to investigate an incident.
